People Caring For And About People. Your Role as a Case Manager:

• Coordinate resident care from admission through discharge planning
• Complete discharge planning assessments within 48 hours of admission
• Develop and manage individualized discharge plans of care
• Serve as a liaison between interdisciplinary teams to ensure coordinated care
• Educate residents and families on diagnoses, medications, and use of medical equipment using the Teach Back Method
• Assist with Advance Directives and ensure appropriate documentation is obtained
• Coordinate insurance updates and manage appeals for denials
• Schedule follow-up appointments and arrange post-discharge services, including DME, medications, and home health
• Ensure all discharge documentation and instructions are complete and communicated clearly
• Conduct post-discharge follow-ups at 1 week and 30 days
• Communicate discharge plans and updates with leadership and care teams
• Supervise unit staff and support adherence to care standards and resident rights
• Assist with activities of daily living as needed
• Maintain a safe, clean, and supportive environment for residents and staff

What We're Looking For:

• Graduate of an accredited School of Nursing
• Current LPN or RN license in the State of Ohio
• Current CPR certification
• 5+ years of experience in healthcare or long-term care preferred
• Strong care coordination, organizational, and problem-solving skills
• Excellent communication and interpersonal abilities
• Ability to work independently and collaboratively with a team
• Compassionate, patient-focused approach to care
• Ability to manage multiple priorities in a fast-paced environment
