Are you a detail-oriented Licensed Vocational Nurse who thrives in a fast-paced, collaborative environment? We are looking for a dedicated professional to join our hospice team as a part-time  Weekend Clinical Support LVN . Hours are: Friday 5:00pm-9:00pm, Saturday 8:00am-9:00pm, Sunday 8:00am-9:00pm.

In this role, you will be the heartbeat of our branch operations, ensuring our patients receive seamless, high-quality care through expert scheduling, coordination, and clinical administrative support.

What You’ll Do

• Coordinate Care: Schedule field visits, prioritize nursing assignments, and ensure all referrals and admits are processed efficiently.
• Operational Excellence: Manage the weekend log, run compliance reports, and facilitate DME (Durable Medical Equipment) orders to ensure patients have what they need, when they need it.
• Clinical Collaboration: Work closely with RNs, Physicians, and the IDT to update patient findings and manage documentation such as DNRs and triplicates.
• Patient Advocacy: Act as a key point of contact for families, providing information, education, and addressing concerns with empathy and professionalism.

Qualifications, Skills, and Abilities:

• Education: Graduate of an accredited school of vocational nursing.
• Licensure: Current State license as an LVN and a valid State Driver’s License.
• Experience: Minimum of two years of experience as an LVN in a clinical setting; hospice or home health experience is strongly preferred.
• Skills: Strong computer proficiency, excellent interpersonal communication, and top-tier organizational abilities.
• Requirements: Reliable transportation and current auto liability insurance; must be able to pass a criminal background check.

Work Environment

• This position involves a hybrid schedule, with duties performed both remotely and in the office.
• The role requires long periods of sitting while conducting computer work.
• You will spend a significant amount of time on the phone and in front of computer screens to manage branch communications and data.

• Quick Decision-Making: This role requires the ability to make rapid decisions and efficiently acquire necessary resources.
• Empathy: You will be responsible for meeting the individualized psychosocial needs of both patients and their families.
• Dexterity: Significant computer time is required, necessitating strong manual dexterity for data entry and navigation.
