A travel opportunity for a Medical Social Worker in Lawrenceville, GA. Start date: 06/15/2026. Duration: 13 weeks. Weekly pay: $1,634–$1,762 per week. Guaranteed hours: 40 per week. Responsibilities
- Conduct psychosocial assessments for inpatients and coordinate discharge planning and follow-up services.
- Assess patient and family needs, identify risks, and arrange appropriate community resources and supports.
- Collaborate with the interdisciplinary care team to develop and implement individualized care plans; document findings in the electronic health record.
- Provide crisis intervention, counseling, and support to patients and families coping with acute illness and transitions of care.
- Facilitate smooth transitions across settings, including referrals to external services and coordination with case management.

Requirements
- Active Georgia Licensed Clinical Social Worker (LCSW) license
- BLS certification (American Heart Association)
- Minimum 2 years of recent experience in medical social work or within the specialty
- Acute hospital experience preferred
- Ability to work in a hospital-based setting and coordinate care with an interdisciplinary team
